Who is Eligible for the Equipment Program?
To be eligible, a person must:
- Be a resident of the State of Oklahoma
- Provide proof of hearing loss
- Provide proof of household income (see application for details)
- Please specifiy if legally blind. This must be verifiable.
- Requests for Captioned Telephones (CapTel) require an audiogram.
